Forms

We've gotten requests for making our required enrollment forms available online-- they are now viewable, but not printable just yet.  I'm working on this, and of course the forms are available in the office, for patients who have already reviewed them and are ready to sign them. 

Patients who have expressed a desire to become members can actually enroll by completing the paperwork-- just call us and we'll guide you through the process. 

We can now accept credit cards, debit cards, and checks.  We are not yet able to arrange for automatic drafts from accounts.

The office has come a long way in a few short weeks.  There are always a few bumps along the way!  We're smoothing them out as we come to them, and are very happy with the new office space.  We look forward to seeing you!
